Membership Form (printable form) - if you are a member of a local riding, show, horse, or 4-H club, please note at the bottom of the form your local club information (name and website or address) and OHSA will give back $5 of your membership to that club. 

To take advantage of our December special simply submit the appropriate fee - $15 for regular adult members and $10 for youth.
